About this Resource
This Leadership Resource was designed to support system and school-based leaders in conducting transition meetings. As students advance through our school system, we aim to connect our staff to foster a cohesive learning environment across school buildings. This resource package includes a Learning Guide, Sample Meeting Agenda, Facilitation Slide Deck, and Planning Documents intended to promote cohesion throughout a K-12 learning environment.
